Employers are encouraged to deal with issues of conduct or performance informally, wherever possible, by the ACAS Code of Practice on Disciplinary and Grievance Procedures. However, it is important to have a record of any informal discussions, in the event that the matter is escalated at a later date to formal proceedings. This form allows you to record in a proper manner the details of any informal discussion, the agreed outcomes, and obtain confirmation from the employee of the conversation.
